A worker died in the early hours of Friday (May 15) at the Starbase facility, a SpaceX manufacturing and launch complex located south of Texas. The incident occurred around 4 am, hours before the scheduled inaugural launch of the Starship V3 mega-rocket. Administração of Segurança and Saúde Ocupacional of the USA (OSHA) is investigating the circumstances of the death.

Autoridades sites have not identified the victim at this time. According to investigations, the person worked for a contractor hired by SpaceX and died after a fall at the facility. The company did not release a public statement about the incident, and the launch of Starship V3 was postponed until the evening of May 21st.

Histórico of injuries in Starbase above average

Dados officials reveal that Starbase records an occupational injury rate significantly higher than other SpaceX units and the aerospace industry average. By 2024, the site had injury rates nearly 6 times higher than comparable space vehicle manufacturing companies and nearly 3 times the average for the aerospace industry as a whole.

Esse padrão de lesões elevadas persiste desde 2019, quando a SpaceX começou a compartilhar dados formalmente com a OSHA. The worrying history reflects intense operating conditions and the accelerated pace of work at Starbase, which has been the epicenter of Starship testing and launches. Analistas apontam que a pressão para manter cronogramas ambiciosos de desenvolvimento pode estar relacionada aos números alarmantes.

Operações intense on Starbase

Starbase serves as the manufacturing, testing and launch nerve center for Starship, the fully reusable rocket developed by SpaceX. The vehicle is more than 122 meters tall and is the largest and most powerful rocket ever built. Recentemente incorporated as a city in May 2025, Starbase gained its own administrative status through a vote (212 to 6).

SpaceX is preparing for a significant milestone: the first test flight of the Starship V3, an advanced and more powerful version of the rocket. Este will be the 12th test flight of Starship, which began flying in its full configuration in April 2023. V3 represents substantial technological evolution, with expanded capabilities for deep space flight and improved reusability.

Cronograma delayed from launch

The inaugural launch of Starship V3 has been postponed twice in recent days. Originalmente scheduled for May 19th, was rescheduled for May 21st at night. SpaceX has not released official reasons for the delays. The fatal incident occurred two mornings before the rescheduled launch, at a critical time in the preparation of operations.

Questões on occupational safety in Starbase gains renewed relevance after death. The site continues to be under regulatory and public scrutiny regarding worker protection protocols:

  • Taxa injury rate 6 times higher than average for space vehicle manufacturers
  • Padrão consistent of incidents since 2019
  • Operações 24 hours accelerated schedule
  • Contratação from multiple contractors for simultaneous projects
  • Ambiente of highly technically complex work

Processos security lawsuits

SpaceX faces legal action related to occupational safety filed by a truck driver. The worker claims to have suffered injuries caused by liquid methane while delivering rocket propellant to the McGregor test site, near Waco, in June 2024. The case exemplifies vulnerabilities in hazardous materials handling protocols in the company’s operations.

Esta action reflects broader pattern of security concerns at SpaceX operating complexes. Reguladores and occupational safety experts closely follow developments at Starbase, particularly as the site combines high-risk aerospace hardware manufacturing with unprecedented operational tempo.

Investigação in progress

OSHA has initiated a formal investigation into the death that occurred at Starbase. The federal agency will examine height safety protocols, personal protective equipment, personnel training and compliance with federal occupational safety regulations. The investigative process can take weeks or months, depending on the complexity of the incident.

Resultados’s investigation may result in citations, fines or demands for operational changes at Starbase. SpaceX has a history of responding to regulatory mandates, although the company often emphasizes the experimental and risky nature inherent in aerospace operations.
