Research shows that metformin, a medication for diabetes, can reduce the progression of vision loss due to age

A recent discovery suggests that metformin, a drug widely used to treat diabetes, may have a significant impact on eye health. Research conducted over five years and published in the journalBMJ Open Ophthalmologyrevealed a possible relationship between the use of the drug and the slowdown in age-related macular degeneration (AMD), one of the main causes of vision loss in the elderly.

The study evaluated approximately 2,000 patients diagnosed with type 2 diabetes and indicated that those who used metformin showed notable benefits. The findings signal a new perspective for treating or preventing the progression of this degenerative eye condition, with a low-cost and highly available medication.

Surprising discovery in vision treatment

Continuous use of metformin was associated with a 37% decrease in the progression of AMD in patients who were already in the intermediate stage of the disease. This statistically relevant reduction was observed even after analyzing and adjusting for several other risk factors that could influence the development of the eye condition, such as age, sex, smoking and other comorbidities.

The results bring a ray of hope to millions of people around the world affected by AMD. Currently, treatment options for the early and intermediate stages of the disease are limited, and intervention is often restricted to the more advanced and aggressive stages, such as wet AMD.

Age-related macular degeneration is a chronic, progressive condition that affects the macula, the central part of the retina responsible for sharp, detailed vision. Over time, AMD can lead to severe loss of central vision, making everyday activities such as reading, recognizing faces and driving difficult.

There are two main forms of AMD: dry (atrophic), which is the most common and progresses slowly, and wet (exudative), which is more aggressive and can cause rapid vision loss due to abnormal growth of blood vessels under the retina. The study focused on the overall progression of the disease, including the intermediate phase, where preventive interventions are most valuable.

Global estimates indicate that AMD affects hundreds of millions of people, making it a growing public health problem. The identification of accessible and effective therapies to slow its progression is, therefore, of utmost importance for the quality of life of the elderly population.

The role of metformin beyond diabetes

Metformin is one of the most prescribed medications globally for type 2 diabetes, acting mainly by reducing glucose production by the liver and improving insulin sensitivity. However, recent research has explored its potential beneficial effects on other health conditions.

Metformin is believed to have anti-inflammatory and antioxidant properties, in addition to influencing metabolic and cellular pathways that may be involved in the pathogenesis of AMD. This ability to act on multiple biological mechanisms may explain its impact on slowing macular degeneration, going beyond its traditional role in glycemic control.

The drug’s accessibility and low cost make it an ideal candidate for future public health strategies. The possibility of reusing an already known and safe drug for a new indication represents a significant advantage in terms of time and resources compared to the development of new drugs.

Future implications and the need for further research

Although the results of this observational study are promising, the researchers caution that additional investigations are needed to confirm the findings. Randomized, controlled clinical trials would be the next step in establishing a definitive cause-and-effect relationship between the use of metformin and the prevention or delay of AMD.

If the benefits are proven in more robust clinical trials, metformin could emerge as a valuable therapeutic option for millions of people at risk of developing AMD or in its early stages, offering a simple and cost-effective way to protect vision and maintain patients’ autonomy for longer.
