Mpox virus detected in Hawaii sewage for the first time
Departamento of Saúde of Havaí confirmed the presence of the mpox virus in a wastewater sample collected at Base Conjunta Pearl Harbor-Hickam on April 13. It is the first detection of the pathogen in sewage in the state. The sample tested positive for clade I virus, a lineage that circulates globally. Autoridades emphasized that no clinical cases have been confirmed in Havaí and that the risk to the population remains low.
The discovery occurred after analysis of the sample, with notification arriving on April 20th and confirmation on April 24th. A second collection carried out in the same period tested negative. The finding serves as an early indicator of possible viral circulation in the community, without confirming active cases or community spread.
Detecção and ongoing monitoring
Health officials continue to monitor major sewage treatment plants on Oʻahu. Todas the recent samples, with the exception of the first, showed negative results. Monitoring continues as an epidemiological surveillance tool to identify possible changes in the viral circulation profile.
The wastewater surveillance strategy has proven effective globally in detecting pathogens before clinical cases emerge. Esse type of monitoring allows authorities to get ahead of potential outbreaks:
- Systematic Coleta in treatment units
- Testes periodic laboratory tests
- Análise trends and variations
- Comunicação with healthcare professionals
- Coordenação with federal agencies
Clade I Características and vaccination recommendations
Clade I is one of the two main lineages of mpox virus in circulation. Ambos clades propagate by the same mechanisms and respond to the same prevention measures. Transmission occurs through direct contact with injuries, bodily fluids or contaminated materials, in addition to prolonged close contact.
Departamento of Saúde recommends that individuals at higher risk of exposure consider vaccination with the JYNNEOS vaccine if they have not yet received both doses of the full schedule. Grupos priority groups include healthcare workers, people with multiple sexual partners, and those who have had confirmed exposure to positive cases.

Orientações for travelers and people with symptoms
Autoridades advise individuals who have recently traveled to areas with active transmission of the virus to monitor their health for 21 days. Qualquer person who presents symptoms such as fever, muscle pain, swollen glands or rash after traveling or contact with suspected cases should seek medical evaluation.
Aqueles who have had close contact with individuals with mpox symptoms should also be aware of possible clinical manifestations. Communication with a healthcare professional is recommended even before symptoms appear if there is confirmed or suspected exposure.
Global Surveillance Contexto
The detection of mpox in wastewater is part of monitoring programs implemented in several countries after the recognition of the public health emergency by the global health body. Havaí follows an established protocol for environmental surveillance of pathogens with potential for dissemination.
The presence of the virus in sewage does not automatically mean symptomatic cases in the community. Estudos show that the virus can be detected in environmental samples even with a low incidence of disease, reflecting asymptomatic infections or residual presence of viral material.
