Cross-contamination inside a commercial kitchen blender caused the fatal allergic reaction suffered by 12-year-old student Mia Shay St Hilaire in south London, an official inquest concluded. Assistant Coroner Briony Ballard formalized the finding at Inner South London Coroner’s Court on Thursday (8 October 2026), bringing the formal investigation into the British schoolgirl’s death to a close.

The student, who resided in Sydenham and had a diagnosed severe allergy to tree nuts, experienced sudden anaphylactic shock after taking only two sips of a Twix milkshake. The appliance used to blend the beverage still held hazelnut residue from a Kinder Bueno milkshake prepared moments earlier in the same machine at Pop Inn Cafe, located on Southwark Park Road in Bermondsey.

Security camera footage recovered from the premises showed that cafe operator Baris Yucel assembled the child’s drink without washing or sanitizing the blending pitcher. The assistant coroner stated in her narrative verdict that the girl suffered an aggressive case of anaphylaxis before losing consciousness at her aunt’s home, where she went into cardiac arrest.

Paramedics rushed the student to Evelina London Children’s Hospital, where intensive care specialists placed her on life support before medical teams turned off the equipment on 30 August 2023 due to irreversible multi-organ failure. The exposure took place on 27 August 2023, when the schoolgirl noticed a distinct taste of nuts immediately after her second sip and alerted her family.

Baris Yucel admitted food hygiene breaches and received eighteen thousand pound fine

A safety inspection carried out by Southwark Council discovered that Pop Inn Cafe had no operational measures in place to prevent allergen cross-contamination during food preparation. Food safety officers also established that the business displayed no warnings or notices informing patrons about potential allergens in menu items.

Criminal proceedings concluded ahead of the formal inquest proceedings. On 2 December 2024, Yucel entered guilty pleas at Croydon Magistrates Court to six charges concerning non-compliance with statutory food hygiene legislation. The magistrate sentenced the business owner on 24 January 2025, imposing a fine of £18,000 alongside an order to complete 100 hours of unpaid community work.

During the formal inquest hearings opened on 5 October 2026, Ballard detailed the rapid clinical progression of the medical emergency. The coroner recorded that the failure to clean the equipment was the direct mechanism that transferred hazelnut traces into the student’s order, resulting in her collapse roughly 25 minutes after consumption.

Family and Natasha Allergy Research Foundation demand dedicated safety rating system

The conclusion of the coroner’s inquiry prompted renewed demands for systemic reform from the student’s relatives and advocacy groups supporting people with severe food allergies. Her parents, Adrian St Hilaire and Chanel Spence, addressed the outcome through a formal statement affirming that Mia was deeply loved and will never be forgotten.

Represented by solicitor Michelle Victor from the law firm Leigh Day, the family coordinated their response with Tanya Ednan-Laperouse, founder of the Natasha Allergy Research Foundation. Ednan-Laperouse established the charity following the fatal reaction suffered by her daughter Natasha in 2016, an event that led to the introduction of Natasha’s Law across the United Kingdom in October 2021.

The charity and the St Hilaire family are urging the Food Standards Agency and local authorities to establish an independent, mandatory allergy management audit scheme. The proposed framework would function alongside standard food hygiene ratings to provide clear visual indicators of allergen handling safety across hospitality venues.
