Republican candidate slams New York attorney general over sharp drop in Medicaid fraud recoveries

A Republican candidate challenging New York’s attorney general has made healthcare fraud enforcement the cornerstone of her campaign, pointing to dramatic declines in money recovered from fraudulent Medicaid claims. Saritha Komatireddy accuses incumbent Letitia James of abandoning aggressive prosecution efforts that once brought hundreds of millions of dollars back to state coffers. The issue has gained national attention following recent scandals involving billions in alleged fraud tied to public assistance programs in multiple states. Federal administrators have now demanded answers from New York about its screening and enforcement practices.

Recovery totals plummet from $168 million to $31 million under current administration

Official data from the attorney general’s annual reports reveals a steep drop in Medicaid fraud recoveries during James’ tenure. The office recovered $168 million in 2019, her first year in office, but that figure fell to just $31 million in 2024. Komatireddy contrasts these numbers with the performance of James’ predecessors, both Democrats and Republicans, who routinely secured far larger recoveries. Former Attorney General Eliot Spitzer’s Medicaid Fraud Control Unit recovered $243.6 million in 2006. Andrew Cuomo, who later became governor, brought in $113.8 million in 2007, $263.5 million in 2008, and more than $283 million in 2009, totaling over $660 million during his first three years as attorney general.

Eric Schneiderman, who succeeded Cuomo, recovered more than $335 million in 2012, marking the second-highest annual total in the unit’s history and its highest recovery in seven years. The challenger emphasized that even James’ Democratic predecessors consistently recovered between $200 million and $300 million annually. Criminal convictions have also declined sharply, according to Komatireddy, who noted that the office once secured around 100 criminal convictions per year. Under James, that number dropped significantly, with one year recording only eight criminal convictions.

State spending on fraud recovery increases while results decline

While recoveries have fallen, New York’s spending on the fraud recovery program has increased substantially. The state allocated approximately $45 million for the program in fiscal year 2020, a figure that rose to $70 million by 2025. The Republican candidate argues this represents a troubling trend of higher costs producing fewer results. She characterizes it as a failure to execute the fundamental responsibilities of the attorney general’s office. Taxpayers face the burden of increased spending without corresponding benefits, Komatireddy contends.

The candidate, who served more than a decade as a federal prosecutor in the U.S. Attorney’s Office for the Eastern District of New York and later as chief of staff at the Drug Enforcement Administration, frames the race as a choice between experienced law enforcement and ineffective leadership. She pledges to strengthen the Medicaid Fraud Control Unit by adding 20 criminal prosecutors if elected.

Federal government demands accountability from New York officials

New York’s handling of Medicaid fraud has attracted scrutiny from federal authorities. Dr. Mehmet Oz, administrator of the Centers for Medicare and Medicaid Services, sent a letter to Governor Kathy Hochul earlier this year requesting detailed information about how the state screens healthcare providers and combats fraud. The review aims to protect Medicaid beneficiaries and maintain public confidence in the program. New York was one of only three states to receive such a letter, alongside California and Minnesota.

The Minnesota scandal, which uncovered billions of dollars in alleged fraud tied to public assistance programs, has elevated Medicaid fraud to a national political flashpoint. The Trump administration has made cracking down on fraud a higher priority, with Vice President JD Vance leading federal enforcement efforts. Republican candidates in races across the country now call for states to intensify prosecution of Medicaid fraud and recover taxpayer money.

Healthcare costs rise as enforcement weakens, candidate argues

Komatireddy maintains that reduced enforcement ultimately harms New York residents by driving up healthcare costs and depleting funds available for other state priorities. When criminals steal from Medicaid without facing consequences, healthcare costs increase for everyone, she explains. The billions of dollars that have gone unrecovered during James’ term represent money that could have been returned to taxpayers instead of requiring additional revenue through tax increases.

State lawmakers continue seeking new ways to extract more money from taxpayers, the candidate observes, when proper prosecution of fraud could eliminate the need for such measures. She characterizes this as a straightforward failure to perform the basic duties of the attorney general position, which includes prosecuting crime and fraud. The candidate describes Medicaid fraud enforcement not as a partisan issue but as a matter of competent administration and protecting public resources.

Campaign emphasizes law enforcement experience versus political priorities

The Republican challenger has built her campaign message around her extensive background in federal law enforcement. Her decade-plus tenure as a federal prosecutor and subsequent role at the Drug Enforcement Administration form the basis of her argument that she possesses the necessary skills and commitment to restore aggressive fraud prosecution. She contrasts her career in law enforcement with what she characterizes as the incumbent’s focus on other priorities at the expense of core responsibilities.

Komatireddy insists that the solution requires simply doing the job of attorney general, which involves prosecuting crime and fraud effectively. Her proposed addition of 20 criminal prosecutors to the Medicaid Fraud Control Unit represents a concrete plan to reverse the declining enforcement trend. The attorney general’s office did not provide a response to requests for comment on the allegations and statistics cited by the challenger.
