Health authorities in Reino Unido and other Hemisfério Norte nations warn of an early start to the flu season in 2025. The scenario is mainly driven by the spread of the subclade K variant of the influenza A(H3N2) virus, which has demonstrated immune evasion capacity.

This specific mutation, identified after the formulation of the annual vaccine, already dominates samples in countries such as Inglaterra and Canadá, resulting in a significant increase in hospitalizations. Registros point to a 56% increase in hospitalizations compared to the previous year, generating concern among professionals in the field.

Health experts emphasize that immunization remains an essential tool to mitigate the risks of serious complications, despite the vaccine’s initial mismatch with the new strain. The expectation is for a potentially more severe season, requiring vigilance and adherence to vaccination campaigns.

Early onset of H3N2 influenza

The 2025 influenza season began weeks ahead of schedule in several regions, including Europa and América of Norte, with cases peaking as early as October. No Reino Unido, flu-related hospital admissions reached 1,717 per day in the last week of November, a number seven times higher than that recorded in 2024.

Recent data from Agência of Segurança Sanitária of Reino Unido (UKHSA) shows that 87% of H3N2 samples analyzed at Inglaterra since August belong to this subclade. No Canadá, similar proportions were observed, with higher detection in children and adolescents aged 5 to 19 years. Especialistas attribute the early progression of the disease to low population immunity against this specific form of the virus, which did not circulate widely in previous periods.

Priority groups for free immunization

The Sistema Nacional of Saúde (NHS) offers the flu vaccine free of charge to populations considered vulnerable, aiming for collective protection and reducing the burden on health services. Essa strategy is essential to contain outbreaks, especially in collective environments where transmission is more facilitated.

Among the priority groups for free vaccination are:

  • People aged 65 and over, due to the high risk of hospitalization and complications.
  • Individuals with chronic health conditions such as diabetes, heart or lung disease.
  • Pregnant women, to protect both the mother and the fetus during pregnancy.
  • Residents of nursing homes and professionals who work as caregivers for the elderly or people with disabilities.
  • Healthcare workers who have direct contact with patients also have access to the dose through their employers. Além In addition, children aged 2 to 11 receive the vaccine in nasal spray format, an effective and less invasive form for this age group. Vaccination coverage increased among pregnant women and young children aged 2 to 3 years compared to 2024, although there was a slight decrease among the elderly.

Current vaccine efficacy against the subclade

The flu vaccine for the 2025-2026 season, formulated with the J.2 strain of H3N2, presents a mismatch with subclade K, which emerged after the selection of strains by Organização Mundial from Saúde (WHO) in February. Testes antigens indicate less reactivity with post-infection sera, suggesting a reduction in virus neutralization.

However, preliminary data from the UKHSA indicates an effectiveness of between 70% and 75% in children against flu-related emergency care. In adults, protection varies from 30% to 40%, focusing mainly on preventing serious hospitalizations and mitigating the severity of symptoms.

Studies conducted in Canadá confirm that 51 of 59 H3N2 samples tested exhibit a reduced response to the current vaccine. Apesar of the mismatch, immunization is still recommended, as it helps to reduce the duration and intensity of symptoms, according to reports from disease control centers. The WHO continues to monitor the evolution of the virus, reinforcing that mutations do not affect the action of antivirals such as oseltamivir.

Guidelines for vaccination in private clinics

Healthy individuals who do not fall into the priority groups for the free vaccine can choose to purchase it from pharmacies and private clinics. The estimated cost of the dose may vary, depending on the location and type of vaccine, but is generally between R$100 and R$150.

Experts recommend the immediate application of the vaccine, considering that the peak of flu cases is approaching, especially with the end of year festivities and the increase in the movement of people. The dose offers season-long protection, with maximum immunity being achieved approximately two weeks after application.

Differences between flu, cold and COVID-19 symptoms

Recognizing the symptoms of H3N2 flu is essential for early management of the disease and to avoid more serious complications, such as pneumonia. Signs of the flu often appear abruptly, in contrast to the more gradual onset seen with common colds.

Influenza infection typically causes a high fever, generalized muscle pain and a feeling of intense exhaustion, which requires complete rest. Tosse persistent dryness and extreme fatigue are common markers of flu presentation, distinguishing it from the nasal congestion and sneezing that predominate in cases of a cold.

In the case of COVID-19, symptoms can be very similar to those of the flu, but often include loss of smell or taste, and in some cases, diarrhea, which may lead to the need for specific diagnostic tests. Home monitoring for mild cases of the flu includes adequate hydration and isolation to prevent transmission, with the recommendation to seek medical attention if there is worsening breathing or other warning signs.

    Additional Viral Prevention Strategies

In addition to vaccination, the adoption of basic hygiene and social distancing measures reinforces the barrier against airborne transmission of subclade K, which has an infection rate of 100 to 140 people per mutated case. Lavar hands frequently with soap and water for at least 20 seconds is a simple and effective practice, capable of removing viral particles and reducing the risk of contamination by up to 20%.

Adequate ventilation indoors is crucial for diluting aerosols, decreasing viral concentrations in the air, especially at family gatherings and workplaces. Regularly cleaning high-touch surfaces such as door handles and switches with approved disinfectants also helps cut chains of contagion.

The use of masks in crowded or poorly ventilated places is additional protection for vulnerable individuals and the community in general, in line with WHO guidelines. Evitar close contact with sick people and considering postponing non-essential travel during infection peaks minimizes collective exposure to the virus.

Global threat from the H3N2 K variant

The K subclade of the H3N2 virus has spread to all continents since May 2025, with detections recorded in regions of Ásia, África and Américas. No Japão, for example, an epidemic was declared in November, marked by a severe outbreak in school environments, highlighting the rapid spread of the variant.

This genetic divergence, which involves eight substitutions in the hemagglutinin, does not appear to increase the lethality of the virus, but increases its contagion capacity in populations with low specific immunity. Na União Europeia, almost 50% of the H3N2 sequences analyzed already correspond to this variant, which represents considerable pressure on local health systems.

Reports from the Centers for Disease Control classify the risk as moderate, but predict co-circulation with other strains, such as H1N1. Continuous surveillance carried out by the global GISAID network ensures crucial updates for the formulation of future vaccines. Nos Estados Unidos, viral concentrations in sewage samples have been rising, signaling an imminent spike in flu cases after the year-end holidays.

Health authorities, such as the CDC, insist on the importance of universal vaccination for individuals from six months of age, even with vaccination gaps. Essa measure aims to mitigate hospital overload and protect the population against the most serious forms of the disease. The 2025 season highlights the need for annual adaptation to viral developments, with a focus on equal access to immunization.
