Polícia Civil of Amazonas concluded the investigation into the death of Benício, aged 6, which occurred in November 2025 in a private hospital owned by Manaus. The final report points out that the child was the victim of a gross medical error by receiving a lethal dose of adrenaline directly into the bloodstream. The medicine should have been administered by inhalation, but the wrong prescription resulted in an overdose. Quatro people were indicted for manslaughter, including the assistant doctor, a nursing technician and two directors of the health unit.

Investigations revealed that Benício’s initial clinical presentation was not excessively severe when he was admitted to Hospital Santa Júlia. The boy sought treatment for a persistent dry cough. Doctor Juliana Brasil, responsible for the first care, prescribed intravenous adrenaline, a substance classified as high alert by the hospital safety protocol. Nursing technician Raiza Bentes performed the application. According to the investigation, the child’s mother even questioned the procedure before application, stating that her son had never received such a drug intravenously, but the warning was disregarded by the health professional.

Dinâmica of failure in hospital care

The error in the route of administration caused an immediate reaction in the child’s body. Poucos Minutes after the injection, Benício began to experience symptoms of shock and severe discomfort. Ele was transferred to the unit’s red room, a sector dedicated to critical cases, where he remained for 14 hours before death was confirmed in Unidade of Terapia Intensiva (ICU). Peritos criminals who analyzed the medical records and the body attested that the condition generated by the injected medication was irreversible.

The expert detailed the central points that led to the fatal outcome:

  • Incorrect Prescrição of adrenaline for intravenous use instead of inhalation.
  • Mandatory safety conference Ausência for high-risk drugs.
  • Aplicação of the drug by the nursing technician even after family questioning.
  • Atitude negligence of the doctor on duty during the patient’s crisis.
  • Falha in direct supervision and security protocols of the hospital board.
  • Confirmação of adrenaline overdose as the primary cause of cardiac arrest.

Conduta of the team and indictments made

One of the most sensitive points in the police report describes the behavior of doctor Juliana Brasil while the patient was in agony. Testemunhas and records indicate that the professional was selling makeup and cosmetic products in the hospital environment at the time the team was trying to stabilize the boy. Para the police authority, this conduct reinforces the thesis of negligence and malpractice, since attention should be fully focused on the immediate assistance of the victim under their direct technical responsibility.

Accountability extended beyond the bedside team. The police indicted two directors of Hospital Santa Júlia because they understood that the organizational structure failed to guarantee barrier mechanisms that would prevent an error of such magnitude. The absence of double checking for high surveillance drugs was cited as a serious administrative gap. The investigation rules out any failure in subsequent intubation maneuvers or in the care provided specifically by the ICU team, focusing the blame on the moment of entry and administration of the drug in the emergency room.

The defense of the professionals involved and the institution has not yet commented in detail on the conclusion of the investigation released last Sunday. The case now goes to Ministério Público of Amazonas, who will decide whether to file a complaint with Justiça. The death of Benício generated strong mobilization in Manaus regarding security in private hospitals and the inspection of medical protocols in pediatrics. The family awaits legal developments to seek redress and the revocation of the professional registration of those involved in the professional councils.
