Headache on its own rarely signals a brain tumor, but pain that changes its pattern deserves a medical exam. According to neurosurgeon Marcelo Vieira da Cunha, from Chapecó (SC), the complaint usually has causes such as migraine, sleep disturbances and muscle tension. Its presence alone, however, is not enough to indicate a lesion. The guidance was first published on 28 October 2026.

Pain patterns that draw medical attention

Physician Pedro Pinheiro (CRM-RJ) states that headache caused by a tumor tends to come with other neurological symptoms. Only about 15% of patients with brain tumors have headache as their sole symptom, and even then only for a short time, since most develop other manifestations within two or three months.

Neurologist Wellingson Paiva points to warning signs: pain that is more intense on waking, pain that does not improve with common medication, or pain concentrated on just one side of the head. The Instituto Nacional de Câncer (Inca) also says loss of strength or touch in the limbs, along with loss of vision or hearing, deserves investigation.

Balance problems and eyesight changes that call for tests

Tumors can affect nerves and areas of the brain linked to vision and coordination. For that reason, double vision, partial loss of the visual field and frequent imbalance may indicate a lesion in the cerebellum or in cranial nerves, explains Paiva. These episodes tend to worsen over time and may appear especially on waking.

The Inca also lists other possible symptoms. The four items below are listed by the institute among the signs to watch.

  • Nausea and vomiting
  • Difficulty keeping balance
  • Changes in behavior
  • Marked drowsiness

Neurosurgeon Arnaout, of Massachusetts General Brigham, adds numbness in the face, difficulty swallowing and difficulty understanding speech to that list.

Seizures in adults call for fast investigation

Seizures also draw attention. According to neurosurgeon Antonio Aversa, head of the Neurosurgery Section at the Inca, epilepsy that starts in adults should be investigated quickly, because it may be a brain tumor. Marcelo Vieira da Cunha reinforces the point: a first seizure should be investigated, especially in young patients.

Warning signs that require immediate care

Not every symptom is an emergency, but some cases require immediate care. According to Pedro Pinheiro, a person should seek urgent care in the following situations:

  • Sudden and very intense headache
  • Loss of strength
  • Difficulty speaking
  • Confusion or fainting
  • Significant change in vision
  • Fever with stiff neck
  • Persistent vomiting

Marcelo Vieira da Cunha warns that sudden loss of strength or slurred speech may indicate a stroke, which is another emergency. Symptoms that advance gradually also need investigation, even without major limitations.

Where the lesion sits matters more than its benign or malignant label

Not every brain tumor is cancer. There are benign and malignant tumors, with different behavior and treatments. Neurosurgeon João Gabriel Ribeiro says the severity of symptoms depends mainly on the area of the brain affected, not only on whether the tumor is benign or malignant.

Imaging exams such as computed tomography and magnetic resonance imaging are used to identify the lesion, according to neurosurgeon Laryssa Azevedo, of Hospital Jayme da Fonte.
